Chronic Care Management (CCM)

We enrolled 115 ACO beneficiaries into our CCM health coaching program. Comparing 2019 and 2020 CMS claims data on these patients, we were average saving of almost $1,200 per patient year over year.

Psychiatric Diagnosis Data

Based on 2020 ACO beneficiary data, patients with high risk diagnoses (congestive heart failure, hypertension, diabetes mellitus/obesity, and COPD) with at lease one psychiatric diagnosis (i.e. anxiety or depression) had up to a 300% greater chance for ER visit and subsequent hospitalization than those in the same high risk categories without a concurrent psychiatric diagnosis. This correlated with an additional $10,000 in cost per patient year or year. The Psychiatric Collaborative Care Management (PsychCoCM) program is designed to improve patient outcomes and, in turn, reduce overall healthcare costs.
